We at DuBois Area School District are fully committed to the health of our students. Our Health Services staff looks after each student’s physical and mental well-being while on our campus.

Administration of Medication

Certain health problems require that students take medication during school hours. Medication must be kept in the health office at all times.

  • A parent/guardian or a responsible adult designated by the parent/ guardian should deliver all medications to the school. We do not permit students to transport any medication to and from school, unless it is an inhaler or EpiPen needed for emergency use by the student and a self-carry form is filed with the school.
  • All student medication must be in the properly labeled bottle or container, as purchased from the pharmacy. Pharmacies will provide an extra properly labeled bottle for school when asked. 
  • No more than a thirty (30) school day supply for any one medication should be stored at school.
  • Parents must complete the required medication permission forms with a doctor’s signature for all medications (prescription and non-prescription). Epi-Pens and Inhalers that are self-carry will also require completion of the DASD self-administration of emergency medication form.
  • Forms are listed under Health Forms, or they may be obtained from the school nurse.

Immunization Information

The Pennsylvania Department of Health Immunization requirements for school entry are as follows:

Children in all grades (K-12) need the following immunizations for attendance:

  • Four doses of tetanus, diphtheria and acellular pertussis (Usually given as DTaP or DTP or DT or Td) - (one dose on or after the 4th birthday)
  • Four doses of polio (4th dose on or after 4th birthday and at least six months after previous dose given)
  • Three doses of hepatitis B (properly spaced)
  • Two doses of measles, mumps and rubella* (usually given as MMR) after age one
  • Two doses of varicella* (chickenpox) after age one or evidence of immunity

    • *Must be given at 28 days apart if not given together

Seventh through 12th grade additional immunization requirements for attendance:

  • Two doses meningococcal conjugate vaccine (MCV)
    • First dose is given 11-15 years of age; a second dose is required at age 16 or entry into 12th grade.
    • If the dose was given at 16 years of age or older, only one dose is required.
  • One dose of tetanus, diphtheria, acellular pertussis (Tdap)
 
Exemptions to the school laws for immunizations are:
 
  • Medical reasons:
  • Religious beliefs; and
  • Philosophical/strong moral or ethical conviction.
 
Written proof of immunizations can be provided by any of the following methods:
  1. Bring immunization booklet to school.
  2. Have your physician fax immunization dates to the school.
  3. Have your physician complete and sign the proof of immunizations form.  Return the form to school.
